Please be advised that the 2012 health premium amounts have been reported on the tax slips twice in error. Once on the pension income slip and again on the taxable benefit slip. We will be mailing out amended 2012 taxable benefit slips today accompanied by the attached letter(English & French attached). The amended slip will exclude the health premium amount. Please note that there are approx. 600 members affected. Our apologies for the oversight and any inconvenience this may have caused to both Allstream and their members.
